program description

This program was developed to assist individuals, 18 years or older, who are eligible for discharge from the Southeast Louisiana Hospital (SELH).

GCTFS helps those individuals reintegrate into their communities and transition
to independent living. We offer Personal Care Assistance (PCA) services and help with housing based on the specific needs of each client.

services provided

• Assistance with personal hygiene, dressing, bathing, or grooming
• Light cleaning tasks in the areas of the home used by the recipient
• Assistance with shopping for health and hygiene products, clothing, and groceries
• Assistance with meal preparation
• Accompanying consumer to the doctor’s office
• Assistance with housing needs
• Assistance with financial planning

The Southeast Louisiana Hospital, located in Mandeville, is a 348 bed state operated psychiatric treatment facility that serves adults, adolescents, and children.

Its Adult Acute Service unit provides comprehensive care for adults with serious
and persistent mental illness, focusing on symptom stabilization, education and discharge planning.
