Long Term Personal Care Services (LT-PCS) is a Medicaid funded program designed to supplement a recipient’s family and/or community supports. LT-PCS enables a person who would otherwise require care in a nursing facility or long-term care facility to remain in his or her own home.

Eligibility is limited to Medicaid recipients 65 years of age and older, or Medicaid recipients who are 21 years of age or older and disabled according to SSA (Social Security Administration) criteria. In addition to these restrictions, the recipients must require assistance with activities of daily living, be able to participate in and direct their care, and meet nursing home facility level of care admission criteria. Services under the PCS program include assistance with activities of daily living (ADLs - eating, bathing, dressing, grooming, transferring, etc.) and instrumental activities of daily living (IADLs - light housekeeping, food preparation, grocery shopping, and laundry).

LT-PCS is limited to 32 hours per week, depending on need.

  • Assistance with activities of daily living (ADL) such as any of the following:
    • Eating
    • Bathing
    • Dressing
    • Grooming
    • Transferring
    • Walking
    • Toileting

  • Assistance with instrumental activities of daily living (IADL) such as the following:
    • Light housekeeping
    • Food preparation
    • Grocery shopping
    • Laundry
    • Reminders about medicines
    • Help with medical appointments
    • Help finding transportation for medical appointments

  • The following assistance is NOT provided:
    • Specialized or skilled nursing
    • Giving medicine
    • Rehabilitative services
    • Help that is already been given by family or through another assistance program
    • Cleaning areas that the applicant does not stay in
    • Food preparation or laundry for anyone other than the applicant
    • Companionship
    • Sitter services
    • Supervision not related to activities of daily living

eligibility

  • Medicaid recipients who are:
    • 65 years of age and older OR
    • 21 years of age or older and disabled according to SSA criteria

  • LT-PCS applicants must also meet all of the following requirements:
    • Meet nursing facility level of care
    • Require at least limited assistance with one ADL
    • Be able to direct their care independently or through a responsible representative

  • LT-PCS applicant must also meet one of the following criteria:
    • Currently reside in a nursing facility and could be discharged if community-based services were available
    • Be likely to require nursing facility admission within the next 120 days
    • Have a primary caregiver who has a disability or is over the age of 70
    • Face a substantial possibility of deterioration in mental or physical condition or functioning if either home and community-based services are not provided in less than 120 days
