This South Central Louisiana Human Services Authority (SCLHSA) program provides a continuum level of care to chronically, mentally-ill consumers in order to help them obtain and maintain independent living in the community.
The GCSS case manager is responsible for providing one-on-one support to each participant, including social skills training, intensive monitoring of housing,
financial management, community integration, medication compliance, treatment compliance (e.g., mental health, substance abuse and health clinic appointments). |

Services include the following:
- Review of housing options
- Access to rental subsidies
- Assistance in managing landlord relations
- Help with developing neighborhood supports
- Learn to administer finances
- Develop socialization skills
- Access to a twenty-four (24) hour crisis intervention
Individuals are also taught basic living skills like:
- Housekeeping
- Meal planning
- Shopping
- Hygiene and personal care
- Home safety best practices

Recipient must be a OMH consumer making regular appointments, and residing in Assumption, Terrebonne, St. Charles, St. James, St. John, or St. Mary parish.
Services are designed specifically to assist consumers in obtaining and maintaining community living. Our goal is to target specific life skills areas, to determine interventions indicated to improve the individual’s functioning, and to facilitate a more independent and successful lifestyle.
